CONFIDENTIALITY AND CONSENT AGREEMENT


Because the main purpose of this request is to help policy makers, the media, and members of the public understand the human consequences of Texas budget cuts, we appreciate your willingness to share your stories publicly. If you request, we will keep the information you provide confidential to the extent that the law allows. If you do authorize the center to use your story in our report, we will need your name and contact information so that we may verify the information you provide. Your name, phone number, and e-mail address will not be searchable in the database.

With your consent, we foresee using the stories we collect in the following ways:

Stories will be posted on our web site in a searchable format, so that you and visitors to our site can read them.

The center will use the information provided in these stories to illustrate the impact of budget cuts and will connect members of the media with people who are willing to share their stories.
